SPRING SPORTS

Athletic permission forms are required.  They should be completed and GIVEN TO THE RESPECTIVE COACH on the first day of practice/tryout.  Do not forward these athletic permission forms to the the school office or nurse.

Physicals need to be current within 13 months of participation and must be valid for the full season.  Physicals may be mailed to the school, attention Nursing Office.  Only one physical form is required for both school and sports.  These forms are available on the Middle School website under FORMS.
