This permission slip should be filled out once during the season and given to Coach Stapleton and Coach Wheeler.  It will serve as a written note of permission for parents to take their daughter from an away game directly.  Once the form is on file with the coaches, parents simply have to tell us they are taking their daughter and please make sure that we visually see you to confirm that your daughter is leaving with you.

If you wish your daughter to go home with another parent from an away game, please send us a separate note giving your permission for your daughter to leave with the designated parent.
