General School Health Information
Physical Exams- Required for ALL students entering K. 6th or prior to grade 7 and 10th grade. The physical exam must be documented on the State Of CT form (HAR-3) and completely filled out by parent/guardian and primary physcian before submitting it to the School Health Office. All incomplete forms will be returned. Completed forms must be submitted to the school nurses.
Immunizations-All entering students must provide documentation of immunizations that are required by the State of CT. Call the school health office in case of medical and/or religious exemptions.
Medication Administration- All students who require medications, prescription or OTC, must have a medication form filled out by their physcian and parent/guardian. All medications must be in their original containers and properly labeled. Students may not carry medication to school. Students who use an inhaler or Epi Pen must also have this form filled out and permission to self carry. All forms must be submitted to the school nurses.
Sports Physicals- Are required every 12-13 months. This physical is not the same as the State of CT physical. It is a more extensive exam to approve your student for sports. NWR #7 follows the CIAC rules which state the student must have all assessments and immunizations required by the State of CT before a sports physical will be accepted. The sports physical form must be completed by the parent/guardian, the primary care provider, and the student athlete and be submitted to the school nurses before the student is allowed to participate in any sports. It is the parents/guardians responsibility to remember when annual exams are due for sports.

Note to Parents/Guardians: We strongly recommend that all medical information, i.e. Health Assessments, sports PE's, medication Admin Forms, etc., be hand delivered to the Health Office due to their personal nature. If you choose to mail, drop off, fax or send these forms in with your student, please be advised that we cannot guarantee the information on these forms will remain confidential and/or they will get to the Health Office where they belong. These forms are secured in your students' health record. Our staff is trained in confidentiality. 
Cell phones are not allowed to be used in the Health Office by students. If your student calls you on their cell, hang up and call the Health Office. They cannot dismiss themselves from school.
